Either the user has forgotten or mistyped the password, or someone is trying to break into the system through this user's account.
Talk to the user to find out whether he or she has forgotten the password. If so, change the password and unlock the account. Otherwise, unlock the account when the user is ready to log in, and take security measures appropriate to your organization.
