Capturing information from respondents is a science in itself, and requires a very structured approach. The design phase of the questionnaire process should involve three important steps:

Clearly define the purpose of the questionnaire. This helps make sure that useful questions are asked and that the questionnaire has a logical flow and keeps respondents interested.

Determine the individual person or group of people that you want to complete the questionnaire.

Create and then test the questionnaire. In the form, click Validate to test whether the questionnaire is assembled correctly. However, the preferred way to test a questionnaire before distributing it is to complete it yourself.
